A sinking concrete sidewalk or driveway can be caused by erosion from rainwater removing the soil under it or a large root beneath it that has rotted over time.

In either case the only way to fix it and prevent it from sinking further is by jacking up and reinforce the ground under it.
Preparing the ground to support a slab that forms a driveway patio or walkway is an essential.
The best way to fix a sunken slab is to inject material underneath it.
Installers drill one or more holes typically two inches in diameter into the sunken section and then inject a slurry of sand cement and water with enough pressure to lift the concrete.
A concrete walkway does not have to be torn apart and poured again to fix this problem.
Before you do that be sure to correct the drainage and soil issues that cause the sinking in the first place.
There is a new method used to fix sinking concrete walkways called mud jacking or slab jacking.
The mud jacking process will make sinking concrete walkways level.
